Understanding Gallbladder Removal Surgery: Procedure and Recovery
Gallbladder removal surgery, also known as a cholecystectomy, is a common procedure to remove the gallbladder, a small, pouch-like organ located in the upper right part of the abdomen that stores bile. How to Use a Disposable Balloon Trocar | SurgSci
A disposable balloon trocar is a medical device used in laparoscopic surgery. It has a blunt tip and an inflatable balloon at the end which is inflated after insertion to create a seal around the incision site and prevent gas leakage during surgery. The device is used to create a port of entry for other surgical instruments during laparoscopic surgery.
Here are the general steps to use a disposable balloon trocar:
1. Make an incision in the skin using a scalpel. The incision should be small enough to allow the trocar to pass through but large enough to accommodate the surgical instrument.
2. Insert the trocar through the incision. The trocar should be inserted at a 90-degree angle to the skin and advanced until it reaches the abdominal cavity.

A trocar is a surgical instrument that penetrates the abdominal wall to provide a passage for other surgical instruments to enter the body cavity and is a minimally invasive surgical instrument. Electrical Lab Tour | Laparoscopic Instruments Factory | SurgSci
In the electrical lab, we have two main devices. One is a power meter, it works for the ultrasonic scalpel system. Another one is a microscope equipment, for measuring our main amplitude and transverse amplitude projects.
In the balance room, we have two analytical balances and one electronic balance. They are used to measure precision reagents. We also have a high temperature blast drying oven, mainly used to measure evaporation residue.
There are also two fume hoods and a water bath. The water bath is used for the configuration of strong acid, alkali reagents and testing. In our gas chamber is a cylinder cabinet, an air generator, a hydrogen generator, a gas chromatograph, a headspace sampler, computers and a printer. We also have equipment for measuring EO residue.

How to Use a Hasson Trocar | Instructions for Use | SurgSci
1. Take the obturator and cannula out of the box using sterile technique.
2. Create an incision and ensure the abdominal cavity has been entered (refer to Hasson abdominal entry skill).
3. Pass two sutures through each fascial edge, assemble the obturator and cannula, insert the trocar into the cavity with suture lie post secured on the cannula.
4. Wrap the suture line around each suture tie post to make the cannula stable on the abdomen, unlock the cam on suture tie post to adjust cannula position in abdominal cavity.
5. Lock the suture tie post, press both locking buttons on the obturator top and release the obturator from cannula.
6. Connect the gas line and open the stopcock to make pneumoperitoneum, then insert surgical instruments through cannula for operation.
7. Push the outer seal release lever on the cannula to release the outer seal for specimen removal procedure during surgery.
8. Remove the gas line, loose sutures on the suture lie post, take out the cannula and suture the cavity.

Based on natural biomacromolecules, quaternary ammonium chitosan (QCS), tannic acid (TA) and oxidant hyaluronic acid (OHA), a new product called QCS-l-TA-l-OHA was developed. Compared with radical resection of rectal tumors, local resection of rectal tumors has the advantages of small surgical trauma, low risk, fast postoperative recovery and short hospital stay, in particular, it is necessary to avoid artificial anus dysfunction and obstacles caused by radical surgery and postoperative voiding function and sexual function.
There are many surgical methods and methods for local excision of rectal tumors, such as transanal approach, transsacral approach, transsphincter approach, and transanal endoscopic approach. They overlap and differ in surgical indications.Due to the different surgical approaches and methods, postoperative complications also vary. 1. Transanal local excision of rectal tumor
This is one of the most ancient and common surgical methods. Its advantages are direct surgical approach, simple operation, no surgical incision on the body surface after operation, and less trauma and surgical risk.
But its disadvantages are:
(1) Due to the position of the patient during the operation, most doctors can only perform surgery in a sitting position, and the surgeon's line of sight and the lesion are in the horizontal position instead of the traditional overlooking surgery that conforms to the surgeon's habits.
(2) The degree of freedom of operation of surgical instruments in the rectum and anal canal is limited, and can only be performed within 45 degrees at most, and many fine operations cannot be performed.

(4) The technical difficulties and shortcomings will become more and more obvious as the distance between the tumor and the anal verge increases.

All of these may ultimately bring some disadvantages and uncertainties to the future surgical efficacy. Surgical indications for transanal resection are therefore usually limited to:
The lesion is located within 4-5cm from the anal verge.
The diameter of the lesion is not greater than 2cm.
If it is a malignant tumor, it must be Tis-T1N0. The more common complications after this operation are postoperative wound bleeding and urinary retention. Accurate mastery of suturing techniques and preoperative prophylactic placement of urinary catheters can prevent the occurrence of these complications.
2. Local resection of rectal tumor through the sacrococcygeal region
Transsacral rectal tumor resection is also known as Kraske's operation; the procedure was pioneered by German surgeon Kraske in 1885.
The surgical steps are briefly described as follows:
The patient was placed in a prone position, and a straight incision was made from the sacrococcygeal joint to the anus. After the coccyx was removed, the pelvic floor muscles were separated to expose the posterior rectal wall, and the posterior rectal wall was incised at the upper edge of the puborectalis to expose the tumor in the rectal cavity. (1) The requirements for the location of the lesion are stricter. The lesion on the anterior wall of the rectum is the best indication. For tumors located on the posterior wall of the rectum or less than 4 cm or more than 7 cm from the anal verge, due to unclear location of the lesion or poor exposure of the surgical field It will make the operation very difficult.
(2) Long-term sacrococcygeal pain is often associated with coccyxectomy, especially when sitting.
(3) The incidence of postoperative wound infection and rectocutaneous fistula is relatively high. A Japanese author reported that the incidence of rectal skin fistula was as high as 20%. The occurrence of the latter will cause greater psychological and physical intrusion to the patient, and the treatment and treatment of this complication are also relatively complicated. In severe cases, a temporary enterostomy is required. This treatment is difficult for patients to understand and accept. Therefore, at present, very few people in the field of colorectal surgery in China use this kind of operation to treat rectal tumors.
3. Local resection of rectal tumor through the anal sphincter
This operation is also known as the Mason operation. Introduced by British surgeon Mason in 1970.
The surgical steps are briefly described as follows; the patient is placed in the prone position. Make a straight incision from the sacrococcygeal joint to the anal edge, cut the skin and subcutaneous tissue, and decide whether to remove the coccyx according to the location of the tumor (when the lower edge of the tumor is greater than 6-7cm from the anal edge, the coccyx needs to be removed). After removing the coccyx, cut off the external anal sphincter For the superficial group and the subcutaneous ring, cut the mesorectum, cut off the puborectalis muscle, expose the posterior wall of the rectum, incise the posterior wall of the rectum from the anal edge to the proximal side, and open and expose the rectum. During laparoscopic procedures and other minimally invasive surgeries (MIS), trocars are used to create small incisions in outer tissue layers. These incisions allow surgeons to insert cannulas through which surgical instruments can be introduced. To ensure optimal access to the operative field (in this case, the pelvis), trocars should be arranged in an arc. Laparoscopic surgery poke hole is the channel for implanting laparoscope and surgical instruments, which is equivalent to the incision of laparotomy, but the choice of poke hole placement is completely different from that of open surgery.
Choice of Puncture Point
1. Observation hole
In terms of histological origin, the tissue structure at the umbilical foramen on the abdominal wall is the weakest, with few blood vessels, and its anatomical layers from outside to inside are skin, thin subcutaneous tissue, rectus abdominis tendon, posterior sheath, and parietal peritoneum.
Therefore, the umbilical foramen is the most suitable site for blind punctures such as observation holes.
For most laparoscopic operations, a puncture hole of 10 mm is often selected in the middle of the umbilical cord or slightly above or below the umbilical cord. If the umbilical foramen is too deep, or if thereis infection, you should choose below the umbilicus. If the pelvic mass is too large, supraumbilical puncture can also be selected.
2. Operating hole
The selection of the puncture point of the operation hole follows the principles of easy operation, aesthetics, and minimal trauma. At the same time, the inferior epigastric artery and other running vessels must be avoided, such as the superficial epigastric artery, deep circumflex iliac artery, and superficial circumflex iliac artery.
The first operation hole: the observation hole.
The second operation hole: the 1-2cm medial side of the left anterior superior iliac spine is the common puncture site.
The third operation hole: at the McBurney point on the right.
The fourth operation hole: The puncture point can generally be outside the midpoint of the connection between the observation hole and the second operation hole, and the distance from the above two operation holes is greater than 8-9cm. Puncture method
1. Trocar direct insertion method
Do not form pneumoperitoneum first, and insert Trocar directly. The technical requirements are relatively high and only suitable for experienced surgeons.
2. Direct-looking trocar puncture method
After the skin is cut at the first Trocar, a laparoscope is inserted into the Trocar, and the incision of each layer of abdominal wall tissue can be observed under the microscope, thus ensuring the safety of the operation.
3. Closed
Enter the Veress Veress needle, and then enter the first Trocar after the pneumoperitoneum is formed. This is a classic puncture mode and is most widely used in clinical practice. However, "blind puncture" can cause damage to retroperitoneal vessels, abdominal wall vessels, and gastrointestinal tract.
4. Open
After incision of each layer of tissue in the umbilicus into the abdominal cavity, a blunt Trocar was inserted to form a pneumoperitoneum. It is suitable for high-risk patients with a history of abdominal surgery or suspected abdominal adhesions, and can also be used for laparoscopic surgery during pregnancy.

1. Laparoscopic appendectomy
Poke a hole on the left side next to the umbilicus as an observation hole, poke a hole horizontally at the level of the navel at the outer edge of the right rectus abdominis as the main operation hole, and poke a hole at McBurney’s point as a secondary operation hole.
Or poke a hole 2 cm medial to the left anterior superior iliac spine as the main operation hole, and poke a hole 2 cm above the pubic symphysis as the auxiliary operation hole.
2. Laparoscopic cholecystectomy
Poke holes at the lower edge of the umbilical cord as observation holes, and poke holes under the xiphoid process, right midclavicular line, and right anterior axillary line as the main, auxiliary operation holes, and assistant auxiliary operation holes.
3. Laparoscopic liver lobectomy
Poke a hole on the upper or lower border of the umbilicus to make an observation hole. The main operation hole is as close to the lesion as possible. If the lesion is in the right liver, take it under the xiphoid process;
The auxiliary operation hole must be kept at a certain distance from the main operation hole and the lens. Generally, the subcostal margin of the right midclavicular line and the subcostal margin of the right anterior axillary line are used, and additional operation holes can be added according to the actual situation.
4. Laparoscopic hepatic cyst drainage
Poke holes at the lower edge of the umbilicus as observation holes, and poke holes under the xiphoid process, at the level of the right midaxillary line at the level of the umbilicus, or below the costal margin of the right midclavicular line as the main and auxiliary operation holes, respectively.

Five Common Tumors In Dogs | SurgSci
Dogs have become an integral part of our lives and many people consider them as family companions. Just like in human medicine, tumors are also a concern for pets. Once dogs suffer from tumors, it can seriously affect their health. Here are five common tumors in dogs:
1. Mast cell tumor
This is one of the most common skin tumors in dogs, accounting for 16-21% of all skin tumors. Mast cells are a type of immune cell mainly associated with allergic reactions and inflammation. Its symptoms are similar to allergic reactions, such as redness and swelling of the surrounding skin, edema, increased vascular permeability leading to bleeding tendency, and may even cause symptoms such as gastric ulcer, hypotension and shock. 2. Lipoma Older

The initial symptoms are fever, loss of appetite, weight loss etc., which are relatively not obvious characteristics of the disease. Generally, when it is discovered, it is in the advanced stage of lymphadenopathy. Lymphoma is one of the few tumors that respond well to chemotherapy. If the dog undergoes chemotherapy in time, there is about 80%-95% chance for the tumor to regress, and 20%-25% of the dog can survive for 2 years; but without chemotherapy, the survival time may be shortened to 1-3 months.
5. Skin Squamous Cell Tumor
This is a common disease in dogs and cats. Cats mainly occur on the head and nasal plane, while dogs often occur on the nasal plane, skin and between the toes.

Luxating Patella in Dogs | Surgsci
Luxating patella means the kneecap moves out of place when the knee bends. It is usually something dogs are born with. A luxating patella can go inside or outside the leg when it pops out of the groove in the thighbone. The kneecap should stay in a groove at the end of the femur. When a dog’s knee moves, the kneecap goes up and down in this groove. But sometimes the kneecap slips out (luxates) in some dogs. Most of the time, a luxating patella in dogs goes inside, or toward the other back leg.

What is a Trocar | The Differences Between Reusable Trocar and Disposable Trocar
A trocar is a medical device that is made up of an awl (which may be a metal or plastic with a pointed or tapered tip), a cannula (essentially a hollow tube), and a seal. Trocars are placed through the abdomen during laparoscopic surgery and function as a portal for the subsequent placement of other instruments, such as graspers, scissors, staplers, etc. Trocars also allow the escape of gas or fluid from organs within the body.

A disposable Pneumoperitoneum Needle is a surgical tool that is used once and then discarded. Cryptorchidism in Cats: Causes, Diagnosis and Treatment | Surgsci
Cryptorchidism is a condition in male cats where one or both testicles fail to descend into the scrotum. The term cryptorchidism comes from the Greek words kryptos, meaning “hidden,” and orchis, meaning “testicle.”
Normally, the testes develop near the kidneys within the abdomen and descend into the scrotum by two months of age. In some cats, this may occur later but rarely after six months of age. If the testicles cannot be felt in the scrotum after two to four months of age, cryptorchidism may be suspected.
In most cases, the undescended testicle is retained in the abdomen or in the inguinal canal, which is the passage through the abdominal wall into the genital region. Sometimes, the testicle may be located just under the skin in the groin region between the inguinal canal and the scrotum.
If the testicle is retained in the abdomen, it cannot be felt from the outside. An abdominal ultrasound or radiographs may be used to determine its exact location but this is not often done before surgery. Typically, only one testicle is retained, known as unilateral cryptorchidism. If a cat does not appear to have testicles but exhibits male behaviors, a hormonal test called an hCG stimulation test can be done to see if he has already been neutered. Alternatively, a veterinarian can check for penile spines which disappear six weeks after neutering.
Cryptorchidism affects less than 2% of cats and may be more common in purebred or pedigreed cats such as Persians, Himalayans, and Ragdolls. It appears to be inherited but its exact cause is not fully understood.
The most common signs of cryptorchidism are male marking behavior (spraying), male cat-associated odors, and aggression. Cats have fewer incidences of testicular cancer and complications associated with cryptorchidism than dogs. One complication is spermatic cord torsion (twisting onto itself) which can cause sudden and severe abdominal pain.
The treatment for cryptorchidism is castration of both testes even if only one has not descended. Surgery involves making an incision into the abdomen or inguinal canal to locate and remove the retained testis.
